Can I donate by check?

Yes! Please mail your check to Care Initiatives at 1611 West Lakes Pkwy., West Des Moines, IA 50266.

Can I specify which location should receive my donation?

Yes. Once you choose to donate by ACH or credit card, you’ll be directed to a secure portal to make your donation, and you may specify which of our skilled nursing facilities or hospice locations should receive your donation.
