How do I know if hospice care is for me?

Our highly experienced and compassionate care team is ready to answer any questions about hospice care. Many patients wait too long, thinking hospice is for the final stages of life, a common myth. Some hospice patients often continue an active lifestyle while receiving personalized care from our Care Initiatives Hospice team. Care Initiatives offers an evaluation and assessment to help decide if and when hospice care may be needed.

Where is hospice care administered?

Care Initiatives Hospice is a service that brings physical, emotional and spiritual care and support to wherever a patient calls home. “Home” may be any of the following: 

  • Patient’s personal residence 
  • Family or loved one’s home 
  • Skilled nursing facility 
  • Assisted living 
  • Hospital 
  • Independent senior living
How is hospice care paid for?

Most people receiving hospice care are covered by the Medicare Hospice benefit. In addition, Medicaid and private health insurance cover hospice services.
