WEST DES MOINES, Iowa (March 10, 2025) — In recognition of their dedication and commitment to preventing the spread of COVID-19, 14 Care Initiatives skilled nursing locations across Iowa have been recognized with Telligen’s Blue Ribbon in COVID-19 Vigilance Award for Q4 2024. This places each facility on Telligen’s 2024 Iowa Blue Ribbon in COVID-19 Vigilance Recognition List.
The 14 locations recognized include:

- La Porte City Specialty Care
- Lamoni Specialty Care

- Parkridge Specialty Care
- Pinnacle Specialty Care
- Ravenwood Specialty Care
- Southridge Specialty Care
“Nursing homes that receive this award have demonstrated their commitment to prevent the spread of COVID-19,” said Tracey Durns, Telligen’s Senior Director Quality Improvement. “We applaud the hard work of these Care Initiatives locations for ensuring that COVID-19 related policies, processes and ongoing staff education are in place and for taking the critical step to get their staff and residents vaccinated.”
Locations receiving the award have met criteria to prevent the spread of COVID-19 for the safety of their residents and staff. This includes completing a COVID-19 preparedness attestation and achieving a resident up-to-date vaccination rate of at least 65%.
