On April 15, 2022, Care Initiatives will host a company-wide National Healthcare Decision Day event from 1:00 to 4:00 PM. All 43 skilled nursing facilities will simultaneously host the event. Six of the seven hospice locations will co-host at a local skilled nursing building, while the Manchester Care Initiatives Hospice office will host independently at their location. 

The event will allow community members the opportunity to complete their advanced directives and/or power of attorney paperwork with a notary. Those interested can stop by any of the 43 Care Initiatives skilled nursing facilities and/or the Care Initiatives Hospice in Manchester, Iowa. Care Initiatives will have available blank copies of advanced directives and power of attorney documents that visitors can complete. Those attending the event should government-issued photo identification so documents can be notarized. 

The advanced directive and power of attorney documents are used by healthcare providers in the event a person is unable to make their own decisions regarding their healthcare, property, or finances. These documents are used to determine a person’s wishes and who they would like to make decisions on their behalf in the event that they are unable to make decisions on their own.  Currently, only 30% of adults have an advanced directive in place. Care Initiatives’ Vice President of Business Development, Tanya Gilyard, expressed her desire to help increase the number of adults with advanced directives and power of attorneys in place to help with future needs. “When one of our residents or patients does not have advanced directives, we see a lot of families face challenges with getting their loved ones access to post-acute care,” said Gilyard. “Our mission as an organization is to help with life’s healthcare transitions. Events like this one allow us to help prepare Iowans for the unexpected, so we can remove barriers to those future transitions.”

While the event is open to the public to assist with getting these documents completed, the staff onsite helping with the event cannot provide guests with legal advice.
